Home health care is medical care provided to patients in their own homes, often by licensed nurses and therapists, focusing on skilled nursing, therapy, and other medical services, as well as assistance with daily living tasks.   Our home health team works in collaboration with you, your family and your Primary Care Practitioner to assist in your recovery, regain your independence, and become as self sufficient as possible.

An individual plan of care will be developed by your Primary Care Practitioner with our Clinical Team, that will allow you to receive the services you need.  Home Health care services we offer include but are not limited to:

In addition, we offer specialized home health programs for patients with COPD, Heart Failure, and Conditions that increase risk of falling.

  • Generally, individuals are eligible for home health care if they are considered "homebound" due to a medical condition, require intermittent skilled nursing care or therapy, and have a Primary Care Practitioner’s order for such services.

Home health care offers numerous advantages, including personalized care, flexibility, family involvement, cost-effectiveness, and the ability to maintain independence and comfort in a familiar environment, your Home. At Home, patients typically recover faster and have fewer complications in the recovery process.

Home health care can benefit a wide range of individuals, including both the young and elderly, those recovering from surgery or illness, people with chronic conditions, individuals with disabilities, and those requiring rehabilitation, allowing them to receive care in the comfort of their own homes.

On your first home healthcare visit, expect a comprehensive assessment of your condition, a discussion of your care plan, and an opportunity to ask questions and address any concerns with the healthcare provider.

We work in unity with your primary care physician, family, and caregivers to determine the services that best suit your needs.  Working as a TEAM allows all members involved to communicate any questions, concerns, and provide information permitting a positive outcome.

Your Home Health Care visits will be based on you personal plan of care.  Your Primary Care Physician my change your personal plan of care as necessary, increasing or decreasing your services and/or the number of visits provided.  Allowing for you to receive the best home health care for your needs.

Yes, you can request a specific home health agency by name.  Federal law gives patients the freedom to choose their health care provider.  We service the following Kentucky counties:  Laurel, Knox, Whitley, and Fayette, as well as the following Tennessee counties:  Anderson, Campbell, Claiborne, Knox, Morgan, Scott, and Union.  Professional Home Health Care welcomes the opportunity to work with you, your family and/or caregiver, and your Primary Care Physician.

Your first home health visit will typically start within 48 hours of a referral, your return home, or on the Primary Care Physician-ordered start of care date.  A staff member will contact you by phone to schedule your first visit.

Home Health Care costs can be covered through a combination of personal funds, private insurance, Medicare and Medicaid, and long-term care insurance.  Medicare pays 100% of the cost of home health care services for eligible patients; Medicare Advantage plans pay according to the policy chosen.

For Home Health Care, "home" generally refers to the patient's place of residence, which can include their own house, an apartment, or even an assisted living facility.

1. You need the help of another person or medical equipment such as crutches, a walker, or a wheelchair to leave your home, or your doctor believes that your health or illness could get worse if you leave your home

2. And, it would require a considerable and taxing effort for you to leave your home and you typically cannot do so.  Note, you can generally leave your home as often as you need for medical treatment that cannot be provided in the home and still be considered homebound.  Additionally, brief absences are allowed for some non-medical reasons such as, to attend church and unique family events but such absences must be infrequent and require a considerable and taxing effort.

No, hospitalization is not a requirement to receive home health care; you can be eligible for home health services even if you haven't been hospitalized, as long as you are homebound and need skilled care.  You can be referred by your Primary Care Practitioner.
